Peak & Bloom does not currently bill insurance directly. Payment is due at the time services are provided.
OUT-OF-NETWORK
BENEFITS
Depending on your insurance plan, you may be eligible for out-of-network reimbursement. Upon request, a superbill can be provided to you to submit to your insurance company. Reimbursement is determined by your individual plan and cannot be guaranteed.
PAYMENT
Current rates and accepted payment methods will be reviewed before your first appointment.
HSA funds may also be used for eligible services.
